Dear Student: If you need any of the forms below, please download the file attached to the link, fill all the information there then send it to the email academic.affairs@fayha.edu.sa . When Academic affairs receives the form and get it signed and stamped, they will send the electronic copy to your email. If you want to get an original signed and stamped copy, then you need to pick it up from the Academic Affairs.
